To purchase an SCA Membership for a specific individual as a gift, please follow these steps:

  1. Print an SCA Membership application form (for a USA/APO address or an International address).
  2. Fill in the recipient’s information to the best of your ability, including their Legal Name and Mailing Address. Please leave the waiver section blank, unless the recipient of your gift can fill it out for themselves.
  3. Payment options: (you can use a Post-it note or separate piece of paper for additional information)
    1. Personal Check – made payable to “SCA, Inc.”
    2. PayPal – request us to generate a Paypal invoice for you. Include the email address of the person paying.
    3. Credit Card – include:
      • The credit card holder’s Name and Billing Address
      • The credit card holder’s email address, if you want a copy of the receipt
  4. Let us know whether you wish this to be an anonymous gift, or would like a note included with the mailed membership card.
  5. Submit the application, payment, and other information listed above, in one of these ways:
    1. Email – scan the form and email it with payment/other details to [email protected]
    2. Fax the completed form and payment/other details to 408-263-0641
    3. Mail the form and payment/other details to SCA Inc, P.O. Box 611928,  San Jose, CA 95161

Once we have received and processed the application, we will send an email confirmation to the recipient. Their membership card will be mailed within 5 business days. If the waiver has not been completed, a waiver form and return envelope will be included with the membership card for them to complete.
