Note: Address changes for Australia must go through the SCAA registry.
Address changes for New Zealand must go through the SCA-NZ registry.
Address changes for Sweden must go through the SKA registry.
Address changes for Finland must go through the Aarnimetsa registry.

This form is only for change-of-address requests. You cannot apply for (or renew) memberships on this form.
